Helping you live with cancer

Improving the Cancer Journey is a local service for people living with cancer

Introducing our service

Being diagnosed with cancer doesn't just affect your physical wellbeing, it can impact on every aspect of your life and the lives of those around you.

Knowing where to turn for support isn't always easy, and we're here to help.

If you live in East Renfrewshire and have been affected by cancer, our Improving the Cancer Journey service is here to support available for you and your family.

The service is delivered in partnership with Macmillan Cancer Support, and has been designed to offer everyone diagnosed with cancer, and their carers, access to the support they need.

Some of the things we can support you with are:

  • Feeling tired, exhausted, fatigued
  • Money and finance
  • Housing, or getting around the house
  • Caring responsibilities
  • Eating, appetite, taste or changes in your weight
  • Worry, fear or anxiety
  • Thinking about the future

The service is about working together to help people with cancer live life as fully as they can.

When you contact us, we'll work with you to determine the right kind of help and support you need.

You'll be assigned a Wellbeing Practitioner who'll meet with you to discuss your needs and help you access tailored information and support.

You can talk to us about how you feel, get help with money and housing worries, emotional support or help at home, whatever matters to you.

Get in touch and we will arrange a chat with a dedicated Macmillan Cancer Wellbeing Practitioner.

The Wellbeing Practitioner will help you complete a Holistic Needs Assessment, a tool used to make sure a person's needs and concerns are identified.

The Macmillan Wellbeing Practitioner will work with you to agree the best kind of support and create a personalised care plan.

You can contact East Renfrewshire Initial Contact Team by phone on 0141 800 7850 or you can email HSCPInitialContactBS@eastrenfrewshire.gov.uk

The service can provide support:

  • Face-to-face in the local community
  • Over the telephone
  • Online via video call

If you're a health or social care professional, you can refer a client.

Once we've received your referral, we'll contact your client and arrange a Holistic Needs Assessment with them.

They can then create a care plan with one of our Macmillan ICJ Wellbeing Practitioners.

You can make a referral by emailing HSCPInitialContactBS@eastrenfrewshire.gov.uk or contact us by phone on 0141 800 7850.

The Cancer Voices group informs, advises and supports the development and implementation of the programme, so that the experiences and needs of people with cancer, their carers and families are taken into account.

If you have personal experience of cancer and would like to get involved in the Cancer Voices project, we would love to hear from you. You don't need specific skills to make an important contribution.
